Ok i had issues when i still had a phat boy gas tube in my 16 inch barrel ar15... i have since gotten a replacement carbine tube but i wanted to post reguarding a saftey concern.
I was shooting 223. ammo some of it was kindve older but what would happen is it would not fire the bullet and the bolt would be stuck in the chamber, i would have to pry it would have to pry it open... the primer had a strike mark on it as well is it a head space issue? I assembled the upper my self.

If the weapon had been performing reliably with full power military ammo, I would suspect that there's something wrong with this .223 ammo.
If you want to experiment with that ammo, be sure to have your ears and eyes on and don't hold onto the magwell.
You get the idea, I hope.
If you had a good primer strike, then the primer is bad. If the casing is really sticking; Do you have a chrome lined chamber? I've seen quite a few non-chrome lined chambers stick due to mild corrosion on the surface of chamber.
It "could" be a headspace issue, but I would check the condition of the chamber first.
There will be a light primer strike when the bolt slams the round in the chamber.
