I am new to AR15's for the most part and I am looking at submitting my application to the atf for a sbr. I have a dpms carbine rifle and I am looking at putting a 11.5 barrel on it. Which form do I need to fill out with the atf to complete this? I don't know if I need to do the form 1 or the form 4? I only need to complete either the form 4 or form 1 and have my fingerprint cards mailed in too. There are no other forms to fill out correct? Also, I have an magpul moe stock on my rifle and would like to know on the form what the length of the rifle will be? Do i put the length of the rifle with the stock fully extended or fully retracted? Does anybody know the length of this setup with a 11.5 inch barrel? When I do get my paperwork back and get my barrel what parts of the rifle do I need to have the information engraved onto? This is my first time doing this so all the help you guys can give would be greatly appreciated.

Do a search on arfcom... several previous threads on the subject.
You need to complete 2 copies of Form 4 only.
You need 2 ATF Fingerprint cards, available from the BATFE site.
You take the 2 completed Form 4's and blank fingerprint cards to your Chief Law Enforcement Officer in your County of residence, the Sheriff.
The fingerprint you and sign both fingerprint cards and both Form 4's then return them to you.
You need to have 2 passport photos of yourself, which are attached to the Form 4's.
Mail all this, along with a check for $200.00 to the BATFE using address on Form 4.
The length of the rifle you put on Form 4 is measured from the end of the stock to the end of the barrel when the stock is closed... the shortest overall length. I listed the OAL of a 7.5" barrel and a CAR Stock for the shortest overall length and listed this on our Form 4's, providing the option of placing a 7.5" on my receiver in the future.
You need your lower engraved, on the bolt-release side, below the factory engraving. ATF has a depth spec.
There is also a way to register the weapon as a Trust, but I'm not sure of the details.

RED should be a Form 1. He is creating the SBR not buying it from a shop. Also the trust is easy as you dont need pictures, finger prints or CLEO signature.
whose name do i put as certifying to citizenship on the certification of citizenship form? And what do most people put for why they intend to make the firearm?
you can SBR a firearm that was already a rifle? I thought it had to be a virgin receiver?

The person applying for the form 1...you.
I put "any lawful purpose" and mine was approved 5 days ago. Really not trying to sound like a dick, but everything you need to know about the process is available on this forum or elsewhere with a search. I was able to read and complete my form 1 by doing the same.
A Form 1 weapon can be engraved anywhere on the receiver which is readily visible. Front of magwell, ejection port side, under trigger, or beside trigger pins.
A Form 4 lower does not need to be engraved at all since it is preregistered with the ATF on a Form 2. The Form 4 is what's used to transfer to you from the dealer. That must be filled out by the dealer and you sign it. If you're using a revocable living trust, it is signed as Your Full Name, Grantor and Trustee.
If you're signing it as an individual, just Your Full Name.
Also the length listed on either the Form 1 or Form 4 is not the shortest length you can legally pin to it. Once you have that stamp you can put any length upper on it. The Form 1/Form 4 configuration is only what the SBR is originally built as.
Lastly, when writing in the Overall Length of the gun, add 19" to your barrel length. For the OP's 11.5" barrel, 30.5" would be the OAL. Some SBRs have folding stocks, and the ATF has said they want a fully extended measurement. This means on your collapsible it would be measured as opened.
Regarding what MountNSlot said: using a Form 4 would be correct if they sold you a registered lower through a dealer. Form 1 weapons are for any lower receiver which you bought privately or through a FFL. You can even make an AR pistol lower into an SBR after you receive the Form 1 back.

You need to know exactly how and why with any of the BATFE forms. All of the questions you will have are answered here in the NFA Class 3 section. Take your time, dig in and make sure you understand all the legalese associated with any NFA item. Yeah a 38 page topic is a bit of a bore but there is useful info in there (for example) Pick your path and have fun.
The one thing you don't want to do is try to take a shortcut. From what I have read it will lead to lots of additional frustration.
