I am working on submitting paperwork for a new form 1 SBR. The manufacturer of the receiver is DC Industries out of St Paul, MN. They are no longer around and I cannot find an address. On the actual form 1 the address is not needed, but on the responsible person (5320.23 question 4b) they ask for it. Anyone have an idea of what the address was? Or how strict the ATF is on having that address?
Here is an example of a past 5320.23 that I've submitted with an AoA receiver that is a Form 1 SBR.

The address should be on the receiver, city and state is all that is needed.
