I have a PSA 16" M4, with the Vltor a5 system installed, with a Springco green spring and running the A5-H2 Buffer. When I run m193 IMI the ejection pattern is around 1 o'clock, if we run any standard 223 ammo like Wolf steel case the pattern is normal around 3 to 4 o'clock, We have tried a new Wilson Combat complete BCG, and have changed out and tried all Vltor A5 heavy buffers with no change to my pattern? what is going on?

I wouldn't consider wolf to be standard ammo, it is usually pretty weak strength ammo but because your upper is very overgassed the ejection is normal with it  l tune my uppers with AGBs to reduce overgassing before trying different springs and buffers but that just me. Good luck.

XM193 is a "spicy" ammo.  It can cause over gassing issues...which would cause your 1 o'clock ejection pattern.  If it runs, it runs.  Adjustable gas block, and turn it down a notch or two. Not a big deal if you don't.
