While looking for a holster on the Falco website I came across a option for Cocked and Locked.  I was not sure what it meant so I sent a question to them - "What is the cocked and locked option mean?".

Dear Morbid_Icon,

Thank you for contacting me.

The Cocked & Locked information applies to holsters that have a thumb break.

If you are going to carry your weapon in holster a Cocked & Locked, we will adjust the thumb break so that the holster fits.

If you do not plan to carry your weapon in holster a cocked & locked, and you choose a holster with a thumb break, please write in the note that you will not carry the weapon in a holster cocked & locked.

If you have any questions, ask.

Yes.  

If you carry clocked and locked, there's a slight difference in height of the thumb strap.  With the hammer back, the strap should be shorter.

Some guns, like the HK usp v1 can be carried in cocked and locked OR DA (hammer down) condition.  

Depending on how you carry that gun, the strap height would be different.
