What would cause a makarov safety to not always de cock when it is placed in the safe position?

Only thing I can think of is a worn tab on the selector or the corresponding tab on the sear

With the lever in the 'safe' position but the hammer still cocked, will the trigger drop the hammer? Also, is the hammer block still in place with the lever on 'safe' and the hammer remaining cocked?
