They have been out of production since 1997. Now that CCW has eased in many states is a form still required for this?Attachment Attached File

By itself, no.
Insert an NAA mini revolver and its an AOW.

Has nothing to do with CCW or state laws on carrying concealed. It's due to National Firearms Act.
