There was a scene in the book The Road that didn't make it to the movie, it was so awful.

The roasted newborn.

You know it's an impactful scene when it creates an entire term.

Bone Tomahawk was rough. No real desire to watch it again.

But Dewey Cox cutting his brother in half with a machete was the worst thing ever filmed.

Irreversable, it genuinely looked like a man had his head caved in with a fire extinguisher.  The camera stays at the same angle the whole scene, there aren't any cuts, it is very graphic.

The fire extinguisher scene in Irreversible is probably one of the most graphic I've ever seen filmed.

ETA: damnit, beaten while searching for clip, which I couldn't find.

The movie High Tension scared the shit out of me as a kid.

If you need a movie to screw your kids sense of well being then rent it for them.

Pan's Labyrinth.  This was a movie about terrifying demons and monsters and the most brutal scene in the movie was the Captain casually beating a man to death with a bottle.  He'd caught a separatist father and son scouting his positions and they had a bag with bread, cheese, and a bottle of wine.  While he was talking to the father he just started tapping the bottom of the bottle on the son's nose and he just kept tapping away harder and harder until he had smashed his nose, then split his face, then crushed his skull.

Hostel II - The bloodbath scene.
In this writer/director Eli Roth slasher film sequel, the most controversial and disturbing death scene was the unsettling
and lingering death of socially-awkward and uptight character of Eastern European vacationer and art student Lorna (Heather Matarazzo).
After Lorna became drunk then kidnapped, she falls prey to Mrs. Bathory, a sadistic wealthy elite who paid to experience the torture and suffering of others.
Lorna is stripped then hung with chains upside-down by her ankles, while Mrs. Bathory (Monika Malacova) enters and removes her black robe to reveal her nakedness.
She reclines below her in an ornately-decorated tub surrounded by candles. Lorna is terrorized as her bare skin is stroked with a large hand-held scythe by her captor.
Bathory cuts Lorna's mouth gag who is swinging helplessly above her naked adorned body.  Lorna (begging for her life) was then repeatedly sliced by Mrs Bathory, who believed
that she would retain her youthful appearance by bathing in the blood of her victim.

Honestly... the modern 'TV series" shouldn't count. They're soap operas, with a semi-unique hook, and there's a new one each week, so they have to try their hardest to be more and more edgy.

Would be more impressived with a movie that is surprisingly brutal at a part, or any other unexpected circumstance.

The most disturbing death scene to me will aways be from 24.

The scene where Jack Bauer puts a gun to the head of Ryan Chappele and murders him to prevent the potential death of thousands from either nerve gas or a bio-weapon. I kept waiting for the reprieve to come and it didn't. The look on Kiefer Sutherland's face kind of stuck with me as he realized that he was going to kill an innocent guy on behalf of a terrorist.

The import of that decision stuck with me.

Came here to say that pretty much word for word. That was 12 kinds of messed up. How they did that I don’t know. There are no detectable clues of it switching from real to CGI or anything.

Also a really messed up rape scene with the same type of cinematography.

In Pans Labyrinth where the guy gets his face smashed in with a wine bottle.

It was so out of the blue it really shocked me and something about it bothered me for days.
