Archived [ARCHIVED THREAD] - Example of Firearm class waiver for military personnel

I was wondering if anyone has a copy or template to be used for the pistol marksmenship waiver form for the mandatory class.  Does this letter need to be signed the the BN CO or if not who then?  Does it have to be notorized?  any and all help is appreciated.

It is much easier to go via your command if they have no issues with doing up a letter.

Something along the lines of this.....
