Under Maker, City and State box for future engraving of the SBR once approved.
Can it be first and middle initials followed by full spelling of last name? Or does your entire full name need spelled out along with City and State? Thanks for your time ??. First eForm 1 here. |

The reply I got from them when I called was Name, not initials city and state.
I engraved my first and last name forgoing my middle. |

It needs to be the name like it is on the Form 1.

As said above, full first and last, middle can be abbreviated for individual, for a trust or other legal entity it's the full name of said entity.
Basically it should match the full name as written in box 3b. for the "Applicant's full legal name". |
