There was actually a pretty huge goof during the end of British active involvement in OIF, as well.

The Brits do have a good excuse, and that is a bureaucracy that makes the American one look risk tolerant and mission command focused.

As to the topic at hand it might be possible that a member of the SAS team got caught with an empty mag and bad guy at contact distance.

Instead of transition to pistol he opted for transition to puddle and rock.
