Leave on a good note.  It's not rare for a person to leave a company for greener pastures and then going back after realizing that they had it good where they previously were.  It happened to me.  

Thank them for everything.  Set a date to go.  Offer to assist with the transition period.  Leave with some class.  You never know when you will need a reference from them.
