For me, Jacob's Ladder.  I really think I felt almost suicidal when it finished.  It put the hook in me like nothing ever will again.  Lots of heavy thoughts about religion, afterlife, etc.  I'm not going to give away spoilers, but if you haven't seen it, see it.  It's for me a movie I love to hate in a way.

Honorable mention goes to The Road.  I was prepared however as I had just read the book, which is even worse.

I have to agree. I watched it for the first time just a few years ago, and was not impressed.

It's no Slaughterhouse-Five, that's for sure.

If you are talking about purely the most disgusting movie ever made I would put Salò somewhere near the top of that list. Italians make some of the most fucked up movies.

Definitely not for the weak of stomach.

The korean movie Isle or Island made an impression with the most gruesome attempted suicide scene ever, along with some undeniably real animal abuse.

The guy who played Lester Ballard gave an amazing performance in one of the most fucked up roles I've ever seen.

It is based on a Cormac McCarthy novel which I didn't know when I watched it but explains a lot of the freaky shit.

The original The Fog scared the shit out of me when I was young.
Communion messed me up when it was new. I have s recurring nightmare if fucking aliens grabbing me.

As did Prince of Darkness. The whole Satan in a jar thing was a mind fuck.

I don’t watch the modern horror movies, think most of them are either needlessly sadistic, of just plain stupid.

Most of those listed here are essentially mainstream in the film world. They’re are widely available and many on streaming services. Really transgressive, disturbing stuff is hard to get for a reason. Sure, Serbian Film is gross, but it’s a slick, mid-tier drama with one truly gross-out scene. Same with Martyrs.

Passolini’s Salo is a bit closer to true transgressive cinema. It’s widely available because Criterion loves it. My stomach can take allot, so not much phases me as long as it’s fiction. The closest things I’ve seen were Men Behind the Sun about Japanese torture in WWII and the Guinea Pig series, which included a fake snuff film that is pretty realistic - so much so that Charlie Sheen (I think) reported it to the FBI. Merhige’s Begotten is pretty nuts. It’s on YouTube.

You literally could not be more wrong, and are very much missing out. The Black Phone is actually a pretty decent movie. And some of the best horror movies have come out in the last 10 years.

Off the top of my head the one that immediately comes to mind is Event Horizon. I'd say the most disturbing though is The Last House on The Left, specifically the 1972 version. I remember watching the full thing on Youtube years back when someone uploaded it in sorta HD. That was an experience...

Titanic - couldn't believe he mooched up to a whale the entire movie only to die by drowning while the whale was beached. Most horrid movie about how far a man can debase himself for a better ride across the ocean and yet still gets the iceberg.
