Likely the EVSE was improperly wired. This has happened even using licensed electricians that do poor quality work such as not torquing screws to spec and using poor quality components. Over time a wire gets loose and starts creating heat.

The thing you plug into the car is just a fancy electrical outlet that tells the car's charger how many amps it is allowed to draw. |
