I'm creating a new post to link to from the main posts about site icons since the old post is archived and can't be edited any longer.

If you would like your ham icon added to your account, send an email to d_j at ar15.com and include your FCC license info (callsign and name), preferably from the email address tied to your arfcom account.

Someone emailed me yesterday from an @yahoo.com email address, but when I replied the mail is blocked. If you asked for an icon and haven’t heard back from me, you may need to try another email address or IM.
