I picked up an OPAP recently and the shot groups are horrible (20" and they're all over the place).  I shoot 10 shot groups and what's odd is 4 or 5 of the shoots are within 5" of each other and the rest are all over the place.  I understand the AK isn't known as being an accurate rifle, but this is bad.  My first thought was that this rifle may not like the GT ammo so I picked up some Tula and the results are the same.  The sights aren't canted and the muzzle crown looks fine.  Other than trying some different ammo, I'm not sure what to do with this.  Any suggestions?

Try checking for loose parts like front or rear sights, the stock, or anything loose on the barrel will cause the pattern to spread.

If possible send it back.  The Opaps I've seen shoot pretty good groups for an AK.  Otherwise you'll have to hunt down the problem yourself.  My first thought is the barrel is shot out or a problem with the crown.
