RRA upper, bolt is ramped using standard hammer with qbuffer. Every once in a while the round chambers but doesn't reset the hammer. What do think is the problem?

Is your standard hammer an AR15 one with a notch? I think most people with ramped bolts use a rounded hammer, DPMS or M16 hammer.
