Could use some help here.

16" mid length 5.56 sight in.
I'm finally at the point where I think something is loose. Been trying to get a consistent string with various ammo.
Here's what I get.. 3-1/2" groups with maybe 2-3 almost touching then one round fly's to left then group settles there for a string of maybe 2-3 then to the right same thing. Then one round fly's to top of target then another to bottom.
I checked the sight and it is holding zero with different rifle.
Get consistent strings with the other rifle and same ammo with lead-sled at same time so it's not a flinch.  
Looked at flash hider and no dings there. So I'm wondering if it's the barrel nut?
Also using a lead-sled for all the sight in work, and like I said have used many different rounds with the same effect.  
Any ideas would be very appreciated.

The bolt face is worn on one side only (compared to others and this was unusual) As well there is a worn away spot right in front of the Lug recess of the upper ( turned upside down it's on the right directly in front of the lug recess)
So does this possibly mean the barrel/upper/bolt&carrier are out of alignment?
I know this would cause some problems.
