Wondering what would be the most common reason for this type of misfeed. Was trying some new ammo (PMC Xtac green tip) with a rifle that's been 100%. This started happening once or twice on every mag. Then it soon started happening on every single round.

In the photo, it appears that the bolt failed to cycle far enough to the rear, preventing the bolt face from getting behind the round to push it forward (feed) into the chamber.  Give the rifle a good cleaning and lube.  With known good mags, load one round per mag and see if the bolt is locked open after firing the single round.  This tests the gas system to see if it's short stroking.

As said,

Test with one round and lock back for the bcg...

Also do a full function chck as well...

Also check that the mag release is screwed in till it's flush on the button...if they are lose sometimes causes wierd issues....
