What light mounts are there that directly mount the light to a KeyMod handguard without adding a rail in between the light and the handguard? If there are any, can any of them quickly detach off of the KeyMod handguard without tools, and without removing the light from the mount first?

I have yet to see a tool-less Keymod mount.  All require some sort of hex or torx wrench.  The system doesn't lend itself well to a tool free QD mechanism.  To do it would be quite a pain.  But for regular Keymod mounts here are a few:

Arson Machine

IWC/HSP Thorntail

Noveske

Arisaka Defense

I think Odin Works had a Keymod light mount either done or in the works.  There's the KAC one listed above, and I think there is another 1 or 2 that I am missing.  Hope this helps some.  All of the companies making these are all top shelf stuff.  Hard to go wrong with any of them.

The Arson one looks pretty good for an offset mount.

Are there any that are not offset, still with no rail? You know, an absolute minimalist mount for mounting directly under the forend for an ambidextrous rig?

The only way to do that would be to get a Surefire Scout like (M300/M600) and run one of the Arisaka scout mounts.  Doing an inline 1" ring mount poses some technical challenges but isn't out of the realm of possibility.  But I don't think there is a ton of demand given that the trend is to want the light above the barrel with most folks.

Thing is, I'm used to an Elzetta mount on my carbine, which has an A2 style FSB to mount it to. The 20" build I'm preparing for will be free floated with a low profile gas block under the handguard, but I'd like to keep the same position for the light when I put one on it. I could just use a rail section and a Surefire-style mount, but I'd prefer to keep it as simple and low profile as possible.

Are you doing this to run a VFG on the bottom side?  Have you thought about running the light in the 11 or 1 o'clock positions?  Put a hand stop, stubby VFG or AFG type thing on the bottom and then the light on top.  Works out really well IMO.  Worth a thought anyway.  I think you are going to have a really hard time getting exactly what you want in a direct attach light mount.  I don't think there is anything currently on the market.  If you are dead set on the 6 o'clock position for the light, probably the best bet and cheapest solution, is to just put a short rail there and get low, 1" scope ring or get a SureFire M300 or M600 and the Arisaka mount.  But you would be looking at well over $200 for that set up if you went with new parts.  Used, you might be able to do better on the light though.
