[ARCHIVED THREAD] - Flat Top Off-Set Dual Mount

I am looking for a dual flat-top rail mount that has a small rail fixed on an angle off the left front side of the rail.

I saw a couple on E-Bay in January, but I can't find one now. Anyone help?
