Why are they being used in semi autos?

Their more solid, have more mass, heavier. Depending on your application, weather you want to be detailed as in retro or want a carrier group that won't fail under a lot of stress.

The bolt carrier has no requirements of a weapon being an actual full-auto, it is just how they are designated. As previously mentioned, there are different factors that can provide benefits to a semi-auto weapon (mass primarily).

Some use full auto BCG with heavier buffers to slow down the cyclic rate, and make the rifle more reliable.

Full auto carrier or Semi auto carrier not much of a difference in weight maybe .8 of an ounce.  You just need a quality Bolt like the BCM.,  158 carpenter steel HP/MPI tested and shot penned.

because semi auto BCG are too light.
I do have one gun i run a SP1 style colt carrier in. the LL ready kind.
You can use your imagination on why.

Are there pics and reports of failed carriers?

I am all about the increased mass, shrouded firing pins, and the reliability of cycling, but I had not seen the reports of the failed carriers.

I have many FA BCGs, but I do own 2 DD SA BCGs which have the shrouded firing pins.  In a 16" midlength, I feel they are right at home, and would operate forever without issue.

Colt started shipping semi-auto rifles with auto carriers years ago.

It makes it simpler, more efficient and less expensive to stock and use a single part.

I like to run FA BCGs in most all of my ARs. Doing so, I usually do not have to run a heavier buffer.

I'm with you on this. I've never seen a quality carrier fail (not counting any gas key issues), and I don't expect to. The chrome RRA BCG in my son's 16" mid works perfectly, and short of a kaboom, we expect it to for a long time.

No need for a full auto bcg in a semi auto, You cant run a semi auto gun fast enough to need one. My semi auto has been flawless in my LMT.

I'd say no real reason other than piece of mind. I cannot say that a semi auto bolt carrier has made my carbine gas AR unreliable or not. I haven't had any cycling issue traced back to the weight of the carrier and I have not had any issue shooting midlength or rifle gas AR's with a S/A bolt carrier either.
