Hey guys I have an eotech 512 and it seems like I can't the damn thing tight enough to stay in one place on the rail. I have it 3 slots in front of the rear sight, I push it forward and tighten with a flat head screw driver. The damn thing wants to move back during fire and if I push it all the way to the rear and tighten it wants to move forward. What the fuck!

It could be the you may have a out of spec rail. Do you have another mil-spec rail that you could try the 512 on?

its a bcm upper receiver. ive tried it on 2 others

Is there a screw designed to keep the mounting hardware from coming loose?

The instructions say to tighten down the hex nut and the retaining screw.

Is yours actually coming loose, or does it simply not tighten down enough?

Actually, they come with a hexnut screw and a flathead mounting screw. you can use one or the other, not both at the same time.

From what I gather is that he will tighten down the screw (feels tight) and it is still loose enough when shooting to force the Eotech back.

Both of the 512's I've used had issues with the flathead retaining screw "stretching."  Once tightened initially, they would loosen up with some use.  Re-torquing them after the first range session fixed them both, but  I found you have to crank down on them pretty good.

I've never used the thumb/ flathead option on mine. Just the hex screw. Never had an issue with it coming loose. I'm guessing you don't have it tight enough or it's a fake. You could try blue loctite but should not need it

What brand is the upper?  A real Eotech has enough threads that it should clamp down on about any rail, even if it's out of spec.

Owning half a dozen Eotech's myself, my money is on it being a fake...

He said its a BCM,it's possible it's the upper but doubtful

It is not a fake I bought it from Bravo company. And I registered it with eotech. I think I am just not tighting it down enough in fear of breaking the hardware but I am a freak like that. I know the threads have stretched. I usually tighten hand tight and then tighten almost a half turn after that. I am using the flat head screw. Maybe I will try the hex.
