Anyone know who makes the muzzle brake on the AR below and where I can purchase one?

That's a short .22lr barrel with a permanently attached vented shroud. It allows a .22lr suppressor to thread onto the muzzle without extending the length by much (essentially a one-stamp non-integrally suppressed SBR).
