Start with more info:

- does it get casing out of chamber or leave it in chamber?
- does it do it with single cartridge loaded in mag or only when trying to load another round after firing?
- in each case, does it do it every time or only some times?
- loading with single shot in mag, does it lock back bolt on empty after firing?

etc

Does bolt stay attached to casing, meaning it doesn't move back at all while it is still grabbing it?

If so, obviously not getting enough (or any) gas back to carrier.

Well, fact that it doesn't lock back with single round,  seems almost certain for whatever reason it's not getting enough gas. Check block for precise alignment, being loose; make sure gas key on carrier is not loose, etc.

If it were locking back every time but still had probs, an extractor upgrade kit might be advisable if it didn't come that way, but doesn't seem to have gotten to the point yet where that might help.
