I assembled the A2 but there was no crush/peel or split-ring washer in the kit. The FH indexed fine; should I still get one, a washer, and install it?

I would not worry about it. If torqued correct what you have is the same as a peel washer with the peel washer. People Pay GS to time muzzle devices so they do not need to use a peel or crush washer.
