     I dont know the proper name for this type of base but the rings and base are all one piece and the lower section of the rings are slanted forward about 35 degrees to help extend the scope farther down the rifle.

   what is the proper name for this type of base or where I can I find one?

    scanned through midway but I didnt have any luck, Thanks, Jon

For magnified optics, it's called a one-piece mount.  To choose one, you need to look at the centerline height (which affects both cheek weld and whether it'll clear a back-up iron sight) and forward offset (which affects eye relief with your preferred cheek weld).  Attachment type is another choice - quick release (levers, like GG&G or LaRue), semi-QR (thumbscrews, like Armalite), or wrench mount.
