Hello,

I'm in process of moving out of state permanently. I have some items on my trust and an SBR is on my name. Just want to confirm that I need to submit two forms, one for the trust and another for individual. Is that right?

Correct, 2 unique forms for 2 different owners (you and your trust).

Also remember you send them in duplicate, so physically you'll be sending 4 forms.
