I've noticed that the 870 with an 18" barrel and extension has a 7 round capacity.

A +2 round extension is 18" OAL.

So my regular 870 Express has a 4 round capacity. If I go to an 18" barrel and attached a +2 mag extension, will I have a 6 or 7 round mag?

The +2 extension (flush with the 18.5" barrel) gives you 6 in the tube, one in the chamber, for a total of 7 rounds total capacity.

Remington's literature can be a little unclear.
