OK... Im struggling here.. lol.. filled out my first form 1... cherry is officially popped.. lol anyway... i have the form filled out.. with signature block on FRONT... and i need to know how to address the envelope to w. va to send it. In the envelope i have (2) copies front and back of form 1 signed by my local chief of police. (2) copies of my figerprints done at the local police station on the blue line fbi card. I have (2) copies of my "Certificate of Compliance" is that all I need to have in the envelope other than a check for $200.00? Other than that... how should i address the envelope to get it there properly and swiftly? Once I have the address.. I will fed ex it if that is appropriate to do so to w. va.
