I am filling out a Form 4 to transfer a Remington 870 Serbu Super Shorty to me.

In box 4. a. do I put in Remington's Info here as the Manufacturer or do I put in Serbu?

No, you're doing a transfer of an already existing firearm.  While Remington may have produced the original receiver, the actual firearm that you are transferring was built by Mark.  It may have Remington 870 in it's ancestry, but it's now a Serbu.

On a Form 1, you are applying for permission to build a firearm, using (in many cases) an existing receiver or firearm.  In that case, BATF wants to know what you're starting with.

Very important, put what is exactly on the Form 3 that your dealer has for that NFA item.  If you put something different it will delay you in the transfer process.

why are you filling out your F4 rather than your dealer?
