Tell them you are the registered owner of SBR serial number 12345 and that you are moving to a new address, 123 Charleton Street, Heston, TX. Include a photocopy of your Form1 or 4 to help them match up your letter with the Registry entry. In you wish, send the letter in duplicate, and ask them to mark one copy as"received" and mail it back to you for your records. If you are moving to a new state, file a 5320.20, and get it back, signed, before you transport it interstate. |

Do a 5320.20 even if you are moving in state. On section 5 put "within State Permanent change of address notification"
I was advised to use a 5320.20 instead of the letter earlier this year. They said they were as much as a year behind on letters. My 5320.20 was returned within 15 days mailbox to mailbox |
