Occasionally my M16 shoots FA when in Semi mode.  

I have replaced the lower parts and have KNS pins with connecting rods so I can't imagine the holes are out of spec unless they are both too low.

Any ideas?  Thanks

FYI, It does it in 9mm, .223 and .22 so I am not thinking it is the bolt.

Sounds like something in the fire control group, since you have ruled out the upper receiver.  If the parts all look right, without any obvious wear signs, try replacing the disconnector springs.  I had this happen to me once.

But then what happens?  Does the bolt close on a live round with the hammer cocked or does the hammer follow the bolt?  I had a trigger group in a new Colt M16 playing up last week and it turned out the disconnector was burred and catching in the trigger channel.  I had another one where the disconnector  spring jammed solid in the trigger channel as well.  If you understand how it works you can eliminate a few things.

Take lower off rifle, put selector on semi, hold trigger back, cock hammer, hammer is now hooked on semi disconnector, do not release it to the trigger sear, keep trigger pulled.  Now grab hammer and wiggle and pull it forward and see if you can get it to slip off semi disconnector, if so there is your problem, fix it. New springs new lower parts, not from cheap vendor.

I checked last night and the hammer does not slip off the disconnecter (I even put a string under it for more leverage), the disconnecter moves freely in the trigger channel and does not seem to bind at all.

The only thing I can think of is that during the violence of an actual shot, the the Extra power Wolf hammer spring has too much power and is forcing itself off disconnecter.  

Is that even possible?  I don't see any obvious wear marks on either the hammer or the disconnecter.

Try an H or HH buffer. Slowing everything down a little may help.  I had a similar problem with an 11.5" barrel.
