I think I am going the Trust route but still on the fence. Assuming I do go with the Trust, would this application be correct? A few questions for those that have successfully filed and received their approval via the Trust route. I understand that having a complete and valid trust is my responsibility. 1. In Box 3b, Did you spell out the "Revocable Living" part of the trust name on the application? States are somewhat different but did anyone name their trust just "XYZ Trust"? Are the "Revocable Living" words required for the trust to be valid? Put whatever your trust name is: IE: Hydguy Trust my street my city ,state 2. If so, Did you engrave your lower with the words "Revocable Living" in the name or just XYZ Trust? What did you name your trust? It has to be the EXACT same as what your trust name is 3. Boxes 10 and 11, leave blank since it is not possible for a legal entity to answer these? 4. With the addition of a signature, would this application be complete and accurate? 5. Certificate of Compliance, not needed because again a legal entity cannot in itself be a citizen?Still need it as you, the grantor, do have to be a citizen To file I would need this form signed by me, in ink, in duplicate and a copy of my Trust and Schedule A? Am I missing anything? yes. 4H needs to have whatever you are engraving on your item, as it is obviously other identifying data Suggestions? Thanks in advance. the name of the trust on the form 1 and the engraving must match exactly what your trust document names your trust.
4H doesn't need anything in it. you can leave it blank. I have one Form 1 with a populated 4H listing the importer's information. it is already assumed that you will engrave it with your name/city/state, pursuant with the GCA ETA: shouldn't it be Knight's MFG, not Knight's MGF |

I just stated in the trust document that it was a revocable living trust. Declaration of Trust Part 1. Trust Name This revocable living trust shall be known as the XYZ Trust. |
