Do I need to fill in all applicable fields on the cards for a Form 1, even Social Sec. #.
I assume the residence field is your address or just city and state?

Yes, you need to fill them all in, particularly the personal identifiers. Height, weight, race, sex, DOB, POB, SSN, and Citizenship. Be sure to sign and date it as well. Leave any of this out, and it will probably be returned to you to add it.
