The Bushmaster owners's manual for the XM15 states that the maximum effective firing rate is (I believe) 45 rounds per minute.

The XM15 can fire much faster than that, so what is meant by effective firing rate?

The 45 rounds per minute comes right out of the operator's manual for the M16A2 Rifle for semiautomatic fire.  Maximum number of aimes shots a minute, incuding magazine changes.  Used for planning purposes only.

Maximum sustained rate of fire, without damage to the rifle or threat of cook offs, is 12-15 rounds per minute.
