| I'm gonna give this a go today. I read this has to be printed double sided with two copies submitted along with fingerprints, photo, and form 5330 assuming CLEO will sign. So I would have two pages instead of the 4 on the printable form correct? page 1-2 front/back then 3-4, with two copies submitted to the WV, address. I'm not even sure I can get the CLEO signatures here so I'll call before I even try. Anything missing? |
| Pages 3 and 4 are instructions for you, so you don't need to print them out. You should read them, though. Otherwise, you are spot on. Two copies of the Form 1 (with pages 1 and 2 back-to-back and the appropriate photos, information and signatures), one 5330, two fingerprint cards and a check for $200. |
