[ARCHIVED THREAD] - Form 1?

When filling out a form 1 has anybody listed there middle initial and if so how is engraved? I just filled out the form and noticed I used my middle initial like I do on all paperwork. Should I change It? I have not got the CLEO signature yet so I can still redo if necessary. Thanks 9c14life
