Does the photo have to be color or can it be B/W?

I don't recall a specification, beyond 2"x2" and taken within the last 6 months.

They didn't have color photography for a good many years after the NFA.  

But if you go get passport pics, they will be in color.

If you are making your own at home with a digital camera, why not print them out in color?

If it doesn't have to be color than I can do it on a laser printer and print it directly in the PDF.

It can be black and white.  Just use common sense.  If it is done with a quality printer, you will have no problem  If it is done with a dot-matrix printer from 1978 and you can't tell the difference between your photo and that of Mickey Mouse, then you might have some issues..!!!    Don't laugh, I have seen some that are THAT bad.
