Some of the questions im not too sure on....please help, yall have already been a big help....im almost there!
1....Question 4a: "Name and location of original manufacturer." this is the manufacturer of my Lower correct? along with city and state? 4b: Type of firearm being made.....In the form 1 instructions it lists numbers with definitions....do i just put the number corresponding with SBR? or just put "short barreled rifle" 4d. Model.......do i put AR 15? 4f...how can i get the overall length of a 10.5" ar15 with retractable stock? ??? 4H..Additional Description......WTF do I put here??? 4i...Why do I intend to make this firearm....seems like a trick question...i really just want one....whats a good acceptable answer? "for recreational purposes" doesnt sound too good i dont think.. 6a...Employer Identification number???? IDKKK what this iss!!! 6b CLass....again WTFFFF 
and do i staple or tape my picturesss to my forms???? and where do my Fingerprint cards gooo?!?!?! These questions are harder than i thought.....FML....please help me

4b short barreled rifle 4d whatever it says as the model on the lower from the factory (for instance, stag arms is stag 15) 4f OAL depends on your stock and can be +/- an inch. The average OAL for a 10.5" SBR is 29.5" 4h blank 4i for all legal purposes is what I put 6a blank 6b blank Picture you can staple, tape or glue if you want.. I use tape sometimes and staples sometimes.

Thank you very much! and since im doing this as an individual, I will have my FIRST/LAST name and CITY, STATE engraved on my receiver only correct? this doesnt go in 4h: additional information? Thanks again!

Well its done,
Mailed the envelope out about 30 minutes ago. Two copies of my Form 1 with photo's stapled, and signed by the Chief of Police, Two copies of my Citizenship form, Two copies of FBI Fingerprint forms One check for 200 dollars made out to the Department of the Justice. And one big prayer to the Big Guy that I filled everything out correctly and its all there..... ...and the wait begins... ->
