I assumed that "Name of Maker" meant my address. Did I assume right?

Also, will the ATF accept fingerprint cards if it has a word error on that has been lined through? I put ATF Form 4 in "Reason Fingerprinted" while I should havee put ATF Form 1.

The line through should be OK as long as it's the proper print cards.
