OK I am about ready to send in a form 4 and just want to make sure I have everything I need:

Check
Form 4 x2
Citizenship form x1
Certification of Trust
Declaration of Trust
Schedule 1

Anything I am missing?

from what i have been reading, it looks like you have anything, but i might be wrong? i might be going that route on monday if my cleo changes his mind and not sign

I sure like just having to print out paperwork, sign it and mail it.


I don't know what I'd do if I had to rely on somebody else signing MY paperwork.....other that the BATFE, of course.

BTW, I already sent in a Form 1 for an SBR and got it back with no issues using quicken Willmaker

and it never hurts to call the ATF in WV to double check, it's worth it.

I sent in:

1x Declaration of Trust
1x Schedule A
1x Assignment of Property
2x Form 4's
1x Certification of Compliance (citizenship form)
1x $200 money order

Probably still on its way to the ATF; I sent it 5/14/2008.

i sent mine in this morning via certified mail, it might be overkill, but what the hell! i got my CLEO to sign off when originally he told me he would not!

If the version of your trust that you send references the schedule A (that the items the trust owns is listed on), you should send in the schedule A, as well.  You don't need to send in both the declaration and certification.  Pick one, and go with it.

this is what i sent in on several form 1's, it is the same and i was always approved:

-Form 1 or 4 (two ink-signed originals)
-Citizenship Form, Form 5330.20 (two ink-signed originals)
-$200 Cashiers check or Money Order
-Copy of Declaration of Trust
-Copy of Schedule A,B,C, with the DoubleStar AR15 in the Schedule A
-Copy of Assignment of Property, adding the DoubleStar AR15 to the trust
-Copy of Certification of Trust

Wow, you guys sure send in alot of unneccesary paper work. Unless something has changed, here is what I sent in both times I got surpressors

1) Completed Form 4 x 2
2) Complete copy of trust paperwork (asset list was blank, but included)
3) $200 dollar tax stamp fee

That is it. I sent in NOTHING else and got approved both times and it was 30days mail box to mail box.

Stupid question time, from a 1st time Form 4'er:  What is this Schedule A that you guys are talking about?  

I'm using a LLC (not a trust) and from what I can gather it is a schedule where your trusts list what NFA items are currently held.  Is that a requirement?  Personally I would not see where the Form 4 requires disclosure of currently owned NFA stuff but then again I'm new to all this.

This is what I intend to submit for a silencer:

2x Form 4
1x Certificate of Compliance
1x $200 check
1x Certificate of Formation (LLC)

Any help is greatly appreciated.  Thx.

Correct.  The Certification of Trust is just that: a certification that the trust actually exists.  If you send the actual Declaration of Trust then the Certification of Trust is just redundant paperwork.

FWIW, I'm hearing a lot about some examiners requiring the actual Declaration of Trust, Schedule A with the transferred item included and the Assignment of Property placing the item into the Schedule A.  I have no personal experience, but it can't hurt to send in the few extra sheets of paper.
