Found here , boxes 1. and 2. seem the same? I'm going the individual route. My understanding is that I just write my name in 1 and 2. Then write USA in 3, and sign my name at the bottom.

It seems to simple, is that it?

Also, I need 2 copies of this and 2 copies of my Form 1, correct?

Yes, Last name, First name, middle initial in box 1 and 2.

United States for box 3.

Sign and date.

One copy of this form and one copy of the trust.

What about the Trust route?

I'm assuming:
1. Trust name
2. My Name (Last, First, MI)

For this form, I wouldn't think you wold need to, but do you need to sign this as a trustee? (like the form 4.) or just as yourself?

Never mind, I just discovered this form does not need to be completed for a trust. I just found this, found on page 5 of an FFL Newsletter by the ATF:

"Items 13, 14, 16, and 17 of the Form 4 do not require completion for a transfer to a corporation or other entity nor is the submission of ATF F 5330.20, Certification of Compliance with 18 U.S.C. 922(g)(5)(B), required."
