My guess would be that it's either photoshopped or cobbled together from someone's parts collection, but if it's an actual production gun I'm not sure how you would aim it.   Take a look at the sights.   It looks like there's a magazine between them, but even if there weren't, they're not in allignment with each other.   The front one's about 1.5" taller.

Similar to an Australian F1 or Italian Villar Perosa/Beretta Moschetto Automatico M1918.  Actually the magazine is canted to the right side so the sights are visible and the rear sight looks like a folding type.  Probably an Italian/Beretta prototype.

It has a dual magazine setup - v shaped.  And it is not photoshopped.

Not S&W, but you can call it the world's first true assault rifle.  It is older, around 1917.

That is not real or it is a one off from a mad machinest with to many guns laying around.

Its real allright.  Designed by Frank Burton and made by Winchester just before we got into World War 1.  Was cancelled because it was too unconventional.

It fired an intermediate cartridge - .345 caliber.
It was select fire.
It had interchangable barrel.
It had an in line stock.
It was 30 years ahead of its time.

the front sight looks similar to an AK, pretty cool
