Just got a Ithaca Model 37, made in 1940. I would need to enter this in my bound book correct? thanks

Yes, it is over 50 years old, therefore it is a C&R, so it goes in the book.

Was it bought using your liscence, or did you buy it at a gun store on a form 4473?
