I recently picked up my new silencer and ran 120 semi auto rounds thru it.
When I got home I stuck a flashlight in the mount end, and looked thru the muzzle.
With the latch at the 12 o'clock position, I noticed that between 1 and 7 o'clock the baffles are smooth.   Between 7 and 1 o'clock, they are not jagged, but not concentric.  The  blast, last baffle and the end cap are perfectly round. All rounds landed center of target, no keyholes.

Could this be a bullet strike, or is this normal?   No debris came out when I shook it upside down.

The bore aperture on the M42K and similar designs like SDN-6 have a stepped cut, such that, looking at the conical baffle from the side, it is not simply cut once across, perpendicular to the bore. It is stepped, so the aperture is not really circular, like you're seeing down the bore. These vents direct gas to that side following the baffle.

Haha, that's not my suppressor cut in half! I can't remember exactly where I found it, but pretty sure it was on silencertalk. I think it was from someone's desk at AAC.

Anyways, there are detailed photos available of the internals of most of the suppressors on the market - AAC, Surefire, Silencerco, YHM, etc. Some are section views like this cut-away, while others show the components disassembled, for suppressors which can be disassembled (not possible for the M4-2K, of course).

Here's an impressive one of an M4-2000 which was inadvertently subjected to an entire magazine on full auto, on a 300 Blackout upper!



This shows that the M4-2000 has seven baffles with much closer spacing than the 762-SD in the previous picture, but it is the same basic fluted conical baffle design, with the step-cut aperture (correct terminology for that feature escapes me at the moment). If you ever wondered how tough AAC's suppressors are, this should pretty well answer that question. Apparently, some number of rounds made it out through the endcap, after having their jackets peeled off on the blast baffle, and then they started piling up in front of the blast baffle, resulting in the mess you can see in the photo.

Baffles 2-6 have a shear cut that is agressive enough that EDM does not clean it up round.  So you're looking at the not round side of those baffle apertures.  

Nothing damaged your suppressor.
