I have last generation of the STG556 before the E4 was introduced. I have shot it about 200rounds in the past without any problems. I had just purchased a Burris XTR 1-4x and taken out to sight in when all the problems started. I was using Federal XM 5.56.  SO in insert the magazine, rac the charging handle, aim and fire and i see that the empty case is still in the chamber and another round is trying to feed with bolt half way locked.   So i take out the magazine, and the empty case is stuck. I use my cleaning rod and just with the weight of the rod the empty case falls out. It definitely was not stuck in there and i did not have to use any force to get it out.  Put the magazine back in and repeat and the same thing happens.  So i take the rifle apart and oil the chamber a bit and look at things once over and change ammo to Privi M193 and this time after the chamber had some oil in it, the empty case comes out of chamber but instead of being thrown out is still sitting in the action and another round had tried to feed. Basically a huge cluster **uck.  

I then just take the magazine out and feed one round in the chamber and it still can not simply extract the round. I had empty cases that either were still left in the chamber that easily were tapped our or empty cases that were rammed back in the around the chamber entrance completely deforming the neck of the cases. I even had a case that was dug in to the polymer part of the stock on the ejection port.  I have taken pics of the rifle, some of how the rounds and cases looked, bolts and extractor.  Does anything look odd? Where should i start? Thanks for all tihe inputs.

It may just be the lighting, but the picture of the extractor looks like there is something odd about the extractor hook.

Have you disassembled the extractor from the bolt ?  I had a 4th Gen. model (not E4) that had extractor problems.  Dave had me check the entire extractor system for the O-ring, which mine did not have.  He sent me a new extractor, spring, pin and O-ring.  Problem solved.  Apparently, MSAR thought it could work without the O-ring, and found out it could not.

Looks like you have a broken extractor claw (been there, done that). Remove the bolt from the bolt carrier. While pressing down on the extractor push the extractor pin out from either side (firing pin will work if you have nothing else). Check the extractor spring hole. If it has a circular indentation around it for the rubber doughnut then you have an old-style extractor that needs to be replaced w/a new style extractor (see pic below).
I've broken a couple of the old style extractors but never one of the new ones.

Same problem I had a while ago, there was a batch of bad extractors at one point IIRC, contact MSARDave and likely he will have you send the rifle back and check it over.

Make sure first that you have the new style extractor as Tomac shows in his post above. Just because you have an STG with no forward assist does not mean you have the new extractor. From what I've been told Dave believes the new extractors were installed around the 68xx serial number.

Alpha let us know whether you had the new or the old extractor. If you had the new extractor let us know what the issue was with it that caused it to break if that can be determined. Thanks.
