Have 2 questions about submitting a Form 4 NFA application through a Trust.

1)  Am I missing anything from my BATF Application?  (See list below)
  - 2 Copies of Form 4 with photos attached to both
  - 1 Copy of Form 5320.23 with photo attached - Responsible Person Questionnaire
  - 1 Copy of Form 5330.20 (Certificate of Compliance)
  - Copy of the Trust with Schedule A
  - FBI Fingerprint Card (x2)
  - $200 - charged to CC on Form 4

I am sending a copy of Form 4 and 5320.23 to my local CLEO.

2) How many headshot photos did you have to submit with your NFA Application?

I went through the paperwork and it seems I have to submit 4 photos.

2 on the Form 4 to ATF
1 on the Form 4 to CLEO
1 on the Form 5320.23 to ATF

Does that sound right to anyone else?  All I know is there were spots on the forms for them in that arrangement.

Just wanted to make sure I have it covered.

What you need to send to ATF for Trust Form 4.

TRUST
(1) Photocopy of Trust Documentation
(1 ) $200 Check to Bureau of Alcohol, Tobacco,
Firearms, and Explosives
(2) Original BATFE Form 4s (5320.4) [Application for Tax Paid Transfer and Registration of Firearm]
(1) Original BATFE Form 5320.23 [National Firearms Act Responsible Person Questionnaire] from all responsible persons
Af x (1) Passport Photo to Item 3e on Form 5320.23 do NOT staple
(2) Original FBI Form FD-258s [Fingerprint Card] from all responsible persons


To CLEO

TRUST
(1) Photocopy of BATFE Form 4 (5320.4) (Application for Tax Paid Transfer and Registration of Firearm)
(1) Photocopy of Your BATFE Form 5320.23 (National Firearms Act Responsible Person Questionnaire)
NOTE: All responsible persons mail a copy of their 5320.23 to their respective CLEO.

So to clarify...
I don't need to attach an additional photo to the 2nd copy to the ATF on form 5320.23?  Why do they have boxes on the Form 4 then?  Wish they were clear about what they need.  I'm glad you said don't staple, because I already did.  I will remove staple and tape I guess.

Also, I believe I have to submit form 5530.20 (Certificate of Compliance).  Probably required for Trust.

The photograph box on the Form 4 or Form 1 is for those filing as an individual, since they don't do the responsible persons form.

And there is no need for anyone to do the cert of compliance form anymore, they rolled those questions into the other forms.

I have another question. Is it ok to use white out on the Form 4? Or at least to white out something and xerox copy it if a signature isn't on the page?  

The firearms store added a name as a trustee that is not a trustee but a beneficiary. They shouldn't be on it.

Just wondering if that would cause a problem.

This.
Strike (so it can still be read) and initial the change. It's the government way. Absolutely do not use whiteout.
