I have now found 3 addresses for sending a form 4. What is the correct address to send the paperwork for converting to an SBR?

To make a SBR, you need a Form 1, not a Form 4.

USPS marks it return to sender, as the offices left those locations more than a year ago and forwarding has stopped.
Send it to the address printed at the top of the form. If two months later you get your forms back marked 'return to sender' you used out-of-date forms, if you get your stamp you sent it to the right address.
