I am transferring a couple of nfa items from one LLC to another LLC and wanted to know do I just leave the second page blank or do I put the name of the LLC in the Transferee blank on question 15?? Thanks for any help.
Also do I just right one check for the lump sum or seperate $200 checks for each form? Thanks |

I sent in two F4s at the same time earlier this year with one combined check for $400 and didn't have any problems because of it. |
